Early Complications after Gastric Surgery

Abstract
We studied postoperative complications on 2605 cases of gastric disease patients who were operated at the Department of Surgery, Kosin Medical College during 15 years from 1973 to 1987.
@ES The results were as follows.
@EN 1) The diseases are: 69.8% in gastric Cancer 13.5% in gastric ulcer, 16.0% in duodenal ulcer.
2) Elective surgery were 2142 cases (82.2%) and emergency surgery were 463 cases (17.8%(
3) Early postoperative specific complications were 153 cases in 130 persons (5.0% in incidences)
4) The distribution of complications were 58 cases of hemorrhage, 28 cases of anastomotic leakage, 27 cases of gastric ou7tlet obstruction. 11 cases of duodenal stump leakage, 10 cases of intra abdominal abscess, 9 cases of small intestinal
obstruction, 2 cases of gastric perforation above the anstomotic ares.
5) The early specific postoeprative complication rate according to the operation type was higher in proximal gastrectomy and primary closure (p<0.05).
6) The incidence of post operative gastro-intestinal nemorrhage was 2.2% (27.6% in mortality).
7) The incidence of duodenal stump leakage was 0.5% (reoperation: 45.5%, mortality: 36%).
8) The incidence of post operative gastro-intestinal anastomotic leakage was 1.1%, and the incidence of post operative esophago gastrostomy leakage (18.5%). esophago-jejunostomy leak age (4.1%) were higher than those of gastro-jejunostomy
leakage
(0.6%) or gastro duodenostomy leakage (0.7%) (p<0.025).
9) The incidence of A-loop obstruction was 0.3% (reoperation: 75%, mortality: 0%)
10) The incidence of gastric outlet obstruction was 1.6% (reoperation: 25.9%, mortality: 0%)
11) The incidence of small intestinal obstruction was 0.3% (reoperation: 100%, mortality: 0%).
12) The early postoperative mortality rate was 1.3% (34 cases): 0.9% (24 cases) in specific complications, 0.4% (10 cases) in on specific complication.
13) The early specific post operative mortality rates according to the disease are 0.8%, gastric cancer, 1.4% in gastric ulcer, 0.7% in duodenal ulcer.
14) The early specific post operative mortality rates: 0.6% in elective surgery, 2.6% in emergency surgery (p<0.005).
